Nominations from clients should now be sent via our Client Nomination URL:

When all information is supplied correctly, the nomination is expedited on the front-end and is entered into the First Health tracking system nightly which prompts packet provision to the nominated physician or facility, with the exception of hospital facilities which should be addressed separately.
